Preparing for Your Orthopedic Appointment

New Patient Paperwork

You will receive a link via mobile or email to
complete your registration and upload your
insurance card and identification.

For your convenience, you may also download, print,
and complete the required patient forms prior to your
appointment.

In addition to the completed forms,please bring the following:

  • Current Government Issued Photo ID
  • Health Insurance Card
  • Referrals (if required by your insurance)
  • Healthcare Payments (Copayments, Coinsurance, Deductibles. Seaview provides our patients the convenience of verifying your insurance coverage ahead of your appointment, but we encourage you to always read and understand your coverage.)

Additional Items to Bring

  • List of all current medications
  • Medical information on your injury or condition
  • Emergency contact information
  • Any previous imaging scans such as X-rays or MRIs (if available)

*You will receive an email to enroll in the portal & download the app. Please do so as this is the most effective way to keep in touch with the practice.

Be sure to wear loose-fitting clothing to your appointment. You may need to adjust
some of your clothing for the physical exam, so be sure to dress accordingly.
